Communication and collaboration in person-centred care planning

<p>Learning disability affects up to 1.5 million people in the UK, about 2 per cent of the population, and people vary widely in their abilities. To ensure they, and their families, receive optimal care, service providers must commit themselves to constant improvement in service delivery. This article discusses an example of individualised, recovery-orientated care for adults with learning disabilities at an assessment and treatment unit in Doncaster.</p>
