Health technology and supported self-care in Liverpool
Telehealth is being used across Liverpool to empower patients to actively manage long-term conditions such as chronic obstructive pulmonary disease (COPD), heart failure and diabetes, and to encourage behaviour change through supported care. Guided by the technology, patients follow a programme at home that includes taking vital signs such as blood pressure, pulse, body weight and oxygen levels. They also watch educational videos designed to increase their confidence in self-care. This article discusses how digital technology is used in Liverpool to support patients’ self-care, examines what role nurses play in service provision and outlines how the service has developed.
